What is the IMPACT Program?
The IMPACT Program is for anyone people with complex chronic health needs including chronic pain, neurological conditions and/or chronic disease.
IMPACT aims to support participants by increasing knowledge, practicing skills, developing goals and implementing a self-management plan to better manage overall health and wellbeing.
IMPACT is available to residents of South Gippsland and Bass Coast who are over 18 years of age and is facilitated by a multidisciplinary team consisting of a registered nurse, Occupational Therapist, Physiotherapist, Dietitian and Social Worker. We also have an Allied Health Assistant supporting the program.
Program Details:
Place: Gippsland Southern Health Service Leongatha Hospital - Primary Health Department
Time: Friday mornings
Length of Program: 8 weeks
Carers are welcome to attend.
Morning tea is provided along with a take home booklet which includes a range of information and an exercise program.
Transport to and from the group can sometimes be arranged.
What to expect when you attend:

1:1 session with clinicians.

Supervised movement and physiotherapy.

Goal setting.

Reflection and relaxation.
How to Access the Service:
Referrals can be made by service providers (GP, shire council, other health professionals). You can also self-refer by contacting our intake team directly.
